To be submitted ALONG WITH your completed film:

1. Your Completed Short Film (maximum 6 minutes)
2. Final Checklist
3. Final Team Roster
4. Team Leader Affirmation
5. Participants Agreement
6. Parent Guardian Release (if one of your team members is a minor)
7. Release for pre-recorded music or original music

Attention - You are also personally responsible for the following releases:

• Locations - you should have these, but most definitely if the locations are private and are recognizable in your shot.
• Owners of any intellectual property - if there are any trademarks, tradenames, personal names, any copyrighted material, etc. which is the focus of any shot in your film you’ll need this.
• Talent - producers may want to get separate talent releases from your cast for your own benefit

The Participants Agreement says that you will have these documents, so your team may be disqualified if you can’t produce these releases later.
